After scheduling an appointment, you will receive a registration link from eDerm.
Login with the patient’s last name and date of birth. If you didn’t receive the email, please check your Spam Folder. If you still can’t find the email or can’t login, please call the office.
If you are a new patient, we ask that you arrive 15 minutes ahead of your appointment.
For Surgical or Cosmetic appointments, you will need to contact our office to schedule.
We participate with many insurance plans, including: Aetna, Anthem BCBS & FEP, CDPHP with exceptions to some plans, Medicare, Highmark, Humana, Empire BCBS, NYS Empire, Cigna, MVP with some exceptions, Tricare (requires prior authorization/ Prime requires a referral
We do not participate with Fidelis, Medicaid, CDPHP Essential Plan, Child Health Plus, CDPHP Medicaid, GHI Emblem health, United Health Care Community Plan, United Health Care Medicare Solutions, Aetna Premier Network, MVP Medicaid, MVP Essential or Marketplace insurance plans.

If your plan requires a Referral to be seen by a Specialist, please obtain it from your Primary Doctor before your visit.
Our online scheduler allows patients to self-schedule up to 90 days into the future. If you are an established patient, you should see a “welcome back!” notice appears after validating your name and date of birth. If you do not see this even though you have been a patient of the practice, please call the office after scheduling your appointment to verify your information, before filling out the registration form which will be e-mailed to you.
